Customizing Your Form Header and Appearance

A well-branded form builds trust and increases submission rates. This guide covers how to customize every visual aspect of your collection form.
Form Header
The form header is the first thing customers see. It sets the tone and tells them what to expect.
Logo
Upload your company logo to display at the top of the form. This instantly tells customers the form is from you, not a random third-party tool.
Tips:
- Use a high-quality image (PNG or JPG)
- Square or horizontal logos work best
- Transparent background PNGs look cleanest
- The logo is displayed at a reasonable size β no need to resize it yourself
Headline
The main title of your form. This is the bold text customers read first.
Good headline examples:
- "We'd love to hear from you!"
- "Share your experience with [Product Name]"
- "Your feedback means the world to us"
- "Tell us what you think"
- "How has [Product Name] helped you?"
Tips:
- Keep it conversational and warm
- Make it about the customer, not about you
- Action-oriented headlines ("Share your experience") outperform passive ones ("Customer feedback form")
Subtext
Supporting text below the headline. Use this to provide context, set expectations, or motivate the customer to submit.
Good subtext examples:
- "It only takes 2 minutes. Your testimonial helps other businesses discover us."
- "We read every single testimonial. Thank you for being part of our journey."
- "Share a quick text review or record a short video β whatever feels natural."
- "Your honest feedback helps us improve and helps others make informed decisions."
Tips:
- Keep it to 1-2 sentences
- Mention how long it takes (reduces abandonment)
- Explain why their feedback matters
Thank You Page
After a customer submits their testimonial, they see a thank-you page. You have three options:
Default Thank You Message
A simple, built-in "Thank you for your testimonial!" message. Available on all plans. Clean and effective for most use cases.
Custom Thank You Page (Pro & Business)
Customize both the title and message of the thank-you page.
Custom title examples:
- "Thank you, you're amazing! π"
- "Your testimonial has been received!"
- "We appreciate your feedback!"
Custom message examples:
- "We'll review your testimonial shortly. Thank you for helping us grow!"
- "Your feedback means a lot to our team. We'll share it on our website soon."
Redirect URL (Pro & Business)
Instead of showing a thank-you page, redirect customers to a specific URL after submission.
Use cases:
- Redirect to a special discount or offer page as a thank-you
- Redirect back to your website
- Redirect to a specific landing page
- Redirect to a survey for additional feedback
GDPR and Consent
If you operate in the EU or serve EU customers, you may need to add consent mechanisms to your form.
Consent Checkbox
Enable a checkbox that customers must check before submitting. The checkbox text is fully customizable.
Example consent text:
- "I agree that my testimonial may be displayed publicly on the website."
- "I consent to the processing of my personal data as described in the privacy policy."
Privacy Policy URL
Link to your privacy policy. The link appears near the consent checkbox, allowing customers to review your data practices before submitting.
Language / Locale
Forms support multi-language configuration. Set the locale to match your audience's language. The default is English.
